To send back by e-mail:

1. Download the BBLCA Values Survey word document by clicking here.
2. Save the document to your computer.
3. Fill in the survey by highlighting the appropriate box and replacing it with an x.
4. Click File -- Save to save the file.
5. E-mail and attach the file to ArkellR@aol.com.
   

To send back by letter mail:

1. Print off a copy of the survey and complete it.
2. Mail the completed survey to:
